Our experienced Transition Support Team works closely with you and all stakeholders to develop a collaborative and coordinated transition plan. We cater to NDIS or NIISQ clients and assist with community reintegration or transfer into their own home in the community, from either Aged Care or Hospitals.
We understand each client will have unique Transition support needs, and therefore, take an innovative and solutions focused approach to achieve a truly personalised outcome for each client. Our team has experience in transitioning clients from hospital, aged care, detention, or change of location. We collaborate, and work inclusively, respecting culture and choice. We assist and support our clients, their families and work closely with Planners, Support Coordinators and Stakeholders to achieve a collaborative transitionoutcome, with collective impact!
Accomodation
Tenancy and Accomodation Support
We offer boutique tenancy and accommodation support services in various locations, providing 24/7 Supported Independent Living or Disability Attendant Care services along with Short (Respite) / Medium Term Accommodation or Individualised Living Options. Our accommodations are designed to meet your specific requirements and provide you with the best possible experience.
Our fully furnished homes come with a welcome pack that includes everything you need to get started, from linens to towels. Plus, you’ll enjoy access to high-speed internet, Netflix, and all utilities, with maintenance covered in your one, weekly rental.
In select areas, we also offer special features like sensory rooms, music rooms, and accessible homes with wet bathrooms. And our experienced Attendant Care Teams are available 24/7 to provide holistic care and support.
With our Professional Complex Care and Clinical Team, including social workers and registered nurses, you’ll have peace of mind knowing that your needs are always taken care of.
